If a case keeps bouncing back after you’ve made changes, here are the most likely reasons:
The rejection reason wasn’t fully addressed. Re-read the comment carefully — sometimes there are multiple issues and all need to be resolved before the case can move forward.
A different item is now causing a problem. Review all line items, not just the one flagged last time. A new issue may have surfaced after your edit.
A required cap code is missing. Some facilities require CAP codes or specific pricing codes on certain items. Not sure what a cap code is? See What is a CAP code and how do I submit bundled items?
An item is not on contract. If an item isn’t on the facility’s active contract, it will continue to be flagged until the issue is resolved on the facility side or the item is removed.
